Home Care

Home…The best place to be.

Horizon Health Home Care is a Medicare-certified home health agency that serves residents in Morrison County and the surrounding areas seven days a week and anytime of day.  Our dedicated home care staff can help you stay safely in the comfort of your own home. 

Home Care is appropriate whenever a person needs assistance that cannot be easily or effectively provided by a family member or a friend on an ongoing basis for a short or long period of time. Some clients receive home care upon returning home from the hospital and some use the services on an ongoing basis.

Horizon Health home based services include:

  • Skilled Nursing
  • Therapy Services
  • Home Health Aides and Personal Care Attendents
  • Homemaking Services
  • Respite Services
  • Licensed Health Care Social Worker
  • Automated Medication Systems

Skilled Nursing includes Registered and Licensed Pracical Nurses providing dressing changes, wound care, post-surgery follow-up, diabetic teaching, IV Therapy and many other services.

Therapy Programs typically include one or more of the following:  physical therapy, occupational therapy and speech therapy, provided by qualified and trained therapy professionals.  Programs are tailored to meet the needs of individuals.

Home Health Aides and Personal Care Attendants assist clients with bathing, exercises and other types of personal cares.

Homemaking provides services such as cooking, cleaning, laundry and grocery shopping.

Respite Services is temporary care provided to an individual in their home.  Respite services is designed to give the primary caregiver(s) a break from their care giving duties.  Respite may be just a few hours or several days in length depending on the plans of the caregiver.

Licensed Health Care Social Worker acts as an adovcate for clients' needs and guides them in making decisions about their physical, financial and emotional care. 

Automated Medication Systems
are available to help people remember to take their medications correctly.
