Horizon Health is expanding our services! Home is the best place to be and our new Independence Plus program is designed to help you do just that - live in your own home as long as possible! If you or a loved one, parent, grandparent, neighbor, or friend need a helping hand our Independence Plus program may be right for you.

Horizon Health Independce Plus Program Includes:

• Housekeeping services
• Meal planning and preparation
  
• Escort to appointments, errands, and religious services
  
• Pick up medications
• Assisting with personal services such as bathing, dressing, and grooming
  
• Assisting with morning and bedtime routines
• Friendly visiting
  
• Companionship
• Respite care
  
• Provide medication reminders
  

Personal services include assistance with walking, eating, grooming, dressing, bathing, incontinence care, and cognitive impairment. Companionship services include arranging appointments, assisting with mail and bill, reading and writing correspondence, care of houseplants, answering the door, crafts and games, supervising home maintenance and much more.

Home Health Aide services are available and include light housekeeping, laundry and ironing, taking out garbage, chaning linens and making beds, organizing and cleaning closets, help with care of pets, and more.
