Assisted Living
Since 1991, Horizon Health has been serving frail adults and seniors with memory care needs through our Harmony Houses.

Independent Senior Living
Horizon Health currently owns or manages facilities in three communities designed to keep seniors independent and active in their hometowns.

Adult Foster Care
Our newest division, United Friends homes provide a place for people with developmental disabilities to live and thrive.

Board and Lodging
We provide a home to men for men with special needs related to psychiatric disabilities. The facility specializes in veteran care.
