ABOUT ADULT DAY SERVICES

Horizon Health Adult Day Services division is part of a non-profit organization committed to providing the highest quality of compassionate and affordable care to dependent adults. Adults we care for may have various forms of dementia or are suffering from a stroke. The Adult Day Services offers professional staff, meals, and activities. Pre-assessments are required to determine a service plan for each individual that is directed at maintaining or improving the participant's capabilities for self-care.

Monday - Friday

8:00 am - 3:30 pm


Cost / Payment

The cost may be covered by private pay, some insurances providers, or waiver funds/county assistance if they qualify. 


Individual Service Plans

The licensing for this day service requires a service plan to be made based on the needs of each person. This means that the attendance will need to be consistent and follow a regular schedule (for example, 3 full days a week, every week, 5 half days a week every week, etc.) It is a full service center where we can help them work on whatever goals are set for their needs, including bathing and hygiene, eating, exercise, socialization, activities, etc.


Personal Care Assistance

Staff will be able to provide whatever cares the individuals need, including incontinence care, bathing (both large accessible tub and shower will be available) and medication administration. Our RN will review their medical needs on at least a monthly basis. The only thing we will not be able to do (for now anyway) is transfers that require a Hoyer lift (though we plan to have a sit-to-stand available for those who may need that assistance).

Daily Schedule 

The daily schedule will include registered dietician-approved mid-morning snack, full lunch, and afternoon snack (depending on the hours they attend). Each day will have physical therapist-approved exercises, social interaction and other fun activities, relaxing activities, and beds and recliners in private rooms available for sleeping if the individual wishes to nap while with us.


Step 1. Fill out Referral Form and send/bring to the Adult                 Day Services facility at 1201 Hillton Rd., Little Falls.


Step 2. We will contact you to set up an assessment to
             make sure that you or your loved one is a good fit.

The Adult Day Services program is funded by a Live Well at Home grant coordinated by Central MN Council on Aging.
