FAMILY CAREGIVER TOOLS
You are doing more than you think.
If you help an older parent, spouse, or friend get through the week, you are a caregiver.
You do not have to do it alone.
Horizon Health is here for you, not just for them.
Supporting seniors 60+ and their caregivers.

Are you a caregiver?
Most people never call themselves one. They think they are just helping. But you might be a caregiver if...
- You manage medications, appointments, or insurance for an older loved one.
- You have noticed memory changes and worry about leaving them alone.
- You feel guilty when you take time for yourself.
- You call or stop by often to make sure they are safe.
- You handle meals, bills, rides, or daily tasks they used to do themselves.
- You are tired, stretched thin, and unsure who to ask for help.
If any of these sound like you, you are a caregiver. That role is one of the most generous things a person can give. It also takes a toll. Caring for yourself is part of caring for them, and that is where we come in.
Being a caregiver is one of the most selfless gifts anyone can give.
We understand that this gift takes its toll on the caregiver and we encourage caregivers to take care of themselves just as much as they take care of their loved one.
At Horizon Health, we understand the needs and importance of taking care of a loved one and we offer many services to the caregivers themselves.
We have education and consulting for caregivers, respite services and support groups to help you along your journey of care giving.
