Remote Patient Monitoring
Tele-Health is Remote Patient Monitoring,which is the electronic transfer of health information that a client enters from the privacy of their own home. This wireless Bluetooth technology does not require additional internet connection and is easy to use.
The Goal of Remote Patient Monitoring : To help seniors live well in their own homes by monitoring vital signs and increasing communication with homecare staff.
Remote Patient Monitoring targets the most common chronic medical conditions such as COPD (Chronic Obstructive Pulmonary Disease), Heart Attack, CHF (Congestive Heart Failure), Pneumonia, Stroke or Diabetes.
It’s Easy to Use! With the Honeywell system in your home, the monitor will provide prompts for you to enter in your vital signs each morning including weight, blood pressure, heart rate, oxygen level, and glucose level.
Your Vital signs are then reviewed each day by a Trained Professional. Clients are notified of any significant vital sign changes outside of the normal parameters.
